Referring Physicians

Whatever your patient needs, we have it here

As a healthcare provider, your patients trust you and your care. Many patients and their families may turn to you for advice on aging well, especially when their health, safety, and memory are affected. Referrals are often a necessary step to ensure your senior patients receive the quality care, time, and attention they need.

Grand Oaks is the right senior living option for your patients. Located on the campus of Sibley Memorial Hospital, Grand Oaks offers personalized senior living care, daily activities, and access to medical services on site.

If your senior patients need senior living care, make Grand Oaks your preferred choice for referrals. Here’s why:

Top-Quality, On-Site Care

Grand Oaks has the same goals for our residents as you have with your patients—delivering the best possible care. Located on the campus of Sibley Memorial Hospital, residents don’t even have to step outside to access Sibley’s emergency, rehab, aquatic, and restorative care services. In the event of an emergency, Sibley Memorial Hospital is right here. Our affiliation with Johns Hopkins Medicine also provides extra resources, consultative services, wellness and prevention programs, and more to residents.

Commitment to Safety

We don’t take our residents’ well-being lightly. We’ve designed our residence for comfort and safety. Patients have access to secure open-air courtyards for resting and walking. There are safety features in each room to alert for immediate help. We closely monitor medications, behavioral changes, cognitive abilities, mood, and more to address our residents’ needs as they evolve. These are all in place to help reduce your patients’ risks for injury and illness as they age.

Senior Living the Way It Should Be

We are committed to caring for the whole person. Our aging well model focuses on the functional and psychosocial wellbeing of every resident. No resident is the same. That’s why we have custom options for senior living, memory care, respite care, and hospice care. As your patients’ needs change, they can always transition from assisted living to memory care or temporary respite care to senior living care.

Short-Term Care Available

Grand Oaks can also help your patients recover from injuries and illnesses, following your post-surgical protocols or treatment plans, for example. Post-operative short-term stays and respite care are available. Temporary residents have access to all the amenities and activities available to our permanent residents while they’re with us.

Enriching, Independent Lifestyle

The emotional and mental health of our residents is just as important as their physical health. We offer stimulating activities that improve our residents’ quality of life. These include mind activities like music and reading groups, physical activities like Tai Chi and ballroom dancing, and social activities like cooking and movie outings. Our residents socialize and form lasting friendships. Our fine dining options may also be catered to meet your patients’ specific nutritional needs.

Compassionate, Trusted Staff

We’ve built a 20-year trusted reputation within our community of providing long-term care that maximizes the dignity, privacy, autonomy, choice, and independence of our residents. It’s only possible through our incredible staff. Our long-tenured staff has years of in-depth senior care experience. We have a nurse practitioner, nursing coordinator, and therapy team on site. Your patients will interact and build relationships with the same people every day.
